You will receive a physical Customer Approval of Transfer to Equitable Advisors letter from your financial professional. This letter asks you to review important information and sign your approval.

Your financial professional will mark (✓) which packet(s) apply to you and your accounts.

Step 2: Review the documents
Open the cover letter and exhibits in the packet(s) indicated on your letter using the links provided below. Carefully read the information in each packet.

Step 3: Sign the letter
Sign the physical Customer Approval of Transfer letter that you received.

Step 4: Return the signed letter
Return the signed letter to your financial professional.
